COVID-19 WAIVER & RELEASE FROM LIABILITY FORM

With health concerns related to COVID-19 FMCA's top priority, all rally attendees are required to sign the COVID-19 Waiver & Release From Liability Form to attend any rally.  Rally organizers, you must use this form.  Failure to use the FMCA-provided form could result in a loss of insurance coverage for your rally.  We thank you for your understanding and cooperation.
